Helping Your Patients Prepare

Each patients transplant journey is unique. Working together, we can help patients be prepared for what’s ahead, and understand their part in becoming a successful transplant candidate.

The Road to Kidney Transplant
  • To be scheduled for evaluation, patients must complete an online medical history questionnaire (MHQ). If they are unable to complete the online form, we encourage them to reach out to their support network for help (family, friends, social workers etc.) As a last resort, we can provide a paper form.

  • We strongly encourage living donation! Having a living donor can dramatically decrease the wait time for a transplant and increase the chance of a successful outcome. 

​

  • The road to transplant can take many years. It is important that patients keep us informed of changes to their information including:

    • Contact information (address, phone number, email)

    • Changes to dialysis unit or schedule

    • Changes to insurance

    • Medical changes (hospitalizations, new health problems)

​

  • It is important that patients adhere to their medication and dialysis regimens.

​

  • It is important to reach and maintain a healthy weight, all transplant candidates must have a BMI of less than 40 for consideration. Regular exercise can also improve a patient’s surgical candidacy.
